Baseline and Bridge

Start only after Summer Lab

Do this baseline in the week after the Summer Field Guide presentation. It is a short placement sample, not an exam and not proof of mastery.

Adult administration rules

• Set aside 30–40 minutes, paper for working, and no calculator.
• Ask Jeremy to work independently. An adult may reread a question exactly, but should not define a term, suggest an operation, point out an error, or teach during the first attempt.
• If support, an interruption, or an accommodation becomes necessary, finish calmly and record the actual conditions in the adult scoring panel. A supported correct answer is useful evidence, but it cannot create a provisional skip.
• Do not open the Grade 4, Fractions, or Grade 5 lessons while the baseline is in progress.
• Submit once, only after every answer and both written responses are complete. The page then freezes and stores that first submission on this browser.

Success criteria

• I can solve a mixed sample without hints.
• I can explain why one method works.
• I can use mathematics to defend a decision in a new context.

Independent practice

Explain and transfer

The assessment includes two unscaffolded written responses. Score them only after the objective first attempt is frozen. The adult rubric measures whether the method is explained and whether mathematics supports the transfer decision; spelling and response length do not change the math score.

The copied record keeps the objective score, the two rubric scores, administration conditions, raw responses, domain results, and next decisions together. The total is descriptive only: use the domain decisions, not one aggregate cutoff.

Delayed check

Run only the delayed check connected to a provisional skip. Do it without reopening this page or showing the first response.

1. Without reopening the lesson, order 0.509, 0.59, 0.095 from least to greatest and round each to the nearest tenth. Explain one comparison.
2. Near Unit 3, prove that 7/20, 0.35, and 35% are equivalent, then compare that value with 1/3.

Score each delayed check as:

• 2: accurate, independent, and explained;
• 1: partly accurate or correct only after a prompt or hint;
• 0: incorrect, unsupported, or not attempted.

Only an independent 2 confirms the named skip. Otherwise use the bridge; do not turn the baseline into repeated test practice.

Evidence and next step

After adult scoring, copy the generated Markdown into a dated file such as jeremy/portfolio/baseline/2026-08-28-grade5-baseline.md. Save Jeremy’s original written working, scan, or photo beside it. Browser storage is a convenience copy, not the portfolio of record.

• Unit 1 repair: revisit the specific gap in Grade 4 decimal introduction or decimal comparison, then begin the Grade 5 bridge.
• Unit 1 provisional skip: this applies to Anchor 1 only. Confirm it with the delayed decimal check before moving to the Unit 1 application or Lesson 2.
• Unit 2 repair: revisit multi-digit multiplication or long division. A two-question sample never skips Unit 2.
• Unit 3 repair: revisit why common denominators matter or adding unlike fractions.
• Unit 3 provisional skip: this applies to Anchor 10 only and must be reconfirmed near Unit 3 with the equivalent-forms delayed check.
• Unit 6 repair: revisit reading and making graphs. The August sample does not skip the Grade 5 data anchors.
• Units 4 and 5: not assessed here; use their own pre-checks when those units approach.

Use jeremy/portfolio/baseline/TEMPLATE.md if the browser copy is unavailable. Never create a filled result or claim a skip before Jeremy’s actual work exists.
